Plans should be kept as simple as possible so that they are clear and, most importantly, so people actually update them as the project progresses. One way to accomplish that is to keep items out of the plan that do not match the scope of the other plan items — Issue Tracker does this for you.

The issue tracking system is part of SharedPlan Central and as such is bundled (as in free) with ProPlanning. It's a web-based service so you can access it from any browser.
